The Diagnostic Pathway
Home Sleep Test (HST)
Portable device you wear one night, measuring airflow, respiratory effort, oxygen saturation, and heart rate. Good for moderate-to-severe obstructive sleep apnea diagnosis. Not ideal for central apnea, mild cases, or complex patients.
In-lab polysomnography (PSG)
Full overnight study in a sleep lab. Measures all HST parameters plus EEG, EMG, EOG, and body position. The gold standard for diagnosing all sleep disorders, including central apnea, REM behavior disorder, periodic limb movement, and complex cases.
Titration study
Follow-up study where CPAP or BiPAP pressures are dialed in during a supervised sleep session. Increasingly done automatically at home with auto-titrating CPAP machines, but in-lab titration is still standard for complex cases.

Making a Diagnostic Trip Work
A workable one-trip pathway:
- Day 1: Sleep medicine consultation. History, physical exam, questionnaire scores (STOP-BANG, Epworth). Decision on HST vs in-lab.
- Day 1 or 2 night: Sleep study (HST at hotel, or PSG at sleep lab).
- Day 2–3: Results interpreted, diagnosis discussed, treatment decision.
- Day 3–4: If CPAP-appropriate: machine fitting, mask trial, initial pressure settings. If oral appliance: impression appointment.
- Day 4–5: Buffer for follow-up, questions, mask adjustment.
A 5–7 day trip covers this with room to breathe. Combining with other medical or dental work stretches it usefully — a full-mouth dental exam during the same week is common.
CPAP machines are TSA-approved carry-on items and don't count toward carry-on limits. If you buy your CPAP in Colombia, bring the original packaging, receipt, and prescription for the return flight. Some patients prefer to buy in the US after diagnosis for warranty and support reasons — that's also fine.
Oral Appliance Therapy (Dental Devices)
For mild-to-moderate obstructive sleep apnea, or for patients who can't tolerate CPAP, custom-made mandibular advancement devices (MADs) are a proven alternative. These are dental prosthetics that hold the lower jaw slightly forward to open the airway during sleep.
Colombian dental sleep medicine is well-established — the same prosthodontists who handle complex restorative dentistry also fabricate these appliances. Two-appointment protocol: impressions on day 1, delivery and adjustment on day 3–4. Cost typically runs US$400–$900 versus US$1,800–$3,500 in the US.

Follow-Up and Supplies
CPAP therapy is a long-term treatment. You'll need mask replacements (typically every 3–6 months for cushions, longer for frames) and filter replacements. Both are available in Colombia at lower prices than US retail, and both can be ordered online in the US too. Many patients do initial diagnosis and machine acquisition in Colombia, then handle ongoing supplies however works for their situation.
The Verdict
Sleep apnea diagnostics is one of the highest-arbitrage medical tourism opportunities that gets almost no press.
